Most sore backs improve with simple care

This page explains why your back may ache and what can help today. Muscles, joints, discs, and ligaments all share the work of bending and standing. Any of them can become sore after more work than usual.

Long sitting, lifting, or one awkward turn may start a flare. Sometimes there's no single event to blame. I'd watch what makes the soreness better or worse during your day.

Daily work can make an older back sore

Discs lose some spring with age, and small back joints can become worn. Those changes are common, even in people who don't hurt. An X-ray won't tell why today is a bad day.

Your own doctor may check how you walk, bend, and raise each leg. The exam may also check strength and feeling in your feet. Ask which finding matters for the care being offered.

Improvement isn't always obvious at first. You may sleep longer or stand up with less trouble. Those gains count, even when some soreness remains.

Gentle movement often helps more than bed rest

Take a short walk, then see how your back feels later. Change chairs before sitting makes you stiff. Don't repeat a chore that brings sharp soreness.

Start again with the amount that went well yesterday. If the next morning feels much worse, cut back a little. Heat may feel soothing, but don't keep using it if your skin hurts.

Get urgent help for new bladder or bowel trouble. Numbness between your legs, around the groin and buttocks, also can't wait. Leg weakness that grows quickly needs prompt care. Fever or soreness after a major fall does too.

  1. A meta-analysis of 33 inception cohorts and 11,166 participants mapped the actual course of low back pain. In acute cohorts, mean pain fell from 52 out of 100 at baseline to 23 at six weeks, 12 at 26 weeks and 6 at one year. In cohorts that already had persistent pain, the same curve started at 51 and only reached 23 at one year. Improvement is fastest in the first six weeks and then slows sharply in both groups.

The Lancet low back pain series states that for nearly all people with low back pain it is NOT possible to identify a specific nociceptive cause; only a small proportion have a well-understood pathological cause such as vertebral fracture, malignancy or infection. Most new episodes recover quickly, recurrence is common, and in a small proportion the pain becomes persistent and disabling - with initial high pain intensity, psychological distress and pain at multiple body sites raising that risk.

The 2017 American College of Physicians guideline makes a STRONG recommendation that acute and subacute low back pain be treated first with non-drug care - superficial heat, massage, acupuncture or spinal manipulation - because most patients improve over time regardless of treatment, and a STRONG recommendation that chronic low back pain be treated first with exercise, multidisciplinary rehabilitation, acupuncture, mindfulness-based stress reduction, tai chi, yoga, motor control exercise, progressive relaxation, EMG biofeedback, low-level laser therapy, operant therapy, cognitive behavioural therapy or spinal manipulation. Drugs are second, and opioids are a weak recommendation of last resort.

  4. The Global Burden of Disease Study 2021 estimated that low back pain affected 619 million people worldwide in 2020 and projects 843 million prevalent cases by 2050. It remains the leading cause of years lived with disability globally, and 38.8% of that disability burden is attributed to modifiable factors - occupational exposures, smoking and high BMI.
