A good treatment claim answers simple questions

A promise needs facts you can use

This page helps you judge what a back treatment may offer. Soreness often changes with time, lighter chores, and better sleep. That can make a treatment seem helpful when several things changed together.

Feeling better still matters. The hard part is knowing what caused it. I'd ask how often people with the same trouble improved.

Clear answers help you decide

First ask which part of your back the treatment is meant to help. A disc, small joint, nerve, or muscle can cause different trouble. One treatment won't fit every cause.

Ask what the exam found and how the care fits that finding. Then ask how long relief may last. One person's result can't answer either question.

Be cautious when someone says a worn disc will be rebuilt. That's a claim needing strong proof. A later X-ray may look different, while the ache eased because you rested or moved differently.

Cost can't be left unclear. Ask what the price includes and whether more visits may follow. Find out what your insurance will pay before booking.

Write down the answers and take time to think. Review them with your own doctor or nurse. Don't let one image matter more than sleep, walking, and daily work.

  1. A meta-analysis of 33 inception cohorts and 11,166 participants mapped the actual course of low back pain. In acute cohorts, mean pain fell from 52 out of 100 at baseline to 23 at six weeks, 12 at 26 weeks and 6 at one year. In cohorts that already had persistent pain, the same curve started at 51 and only reached 23 at one year. Improvement is fastest in the first six weeks and then slows sharply in both groups.

  2. A meta-analysis of 11 cohort studies found that lumbar disc herniations spontaneously RESORB in about two thirds of conservatively treated cases (pooled incidence 66.66%), documented on repeat CT or MRI. A herniated disc on a scan is not a permanent structural verdict, and the natural course is a confounder in every uncontrolled report of a disc procedure.

  3. A multicentre randomised trial assigned 169 surgical candidates with symptomatic lumbar spinal stenosis to surgical decompression or physical therapy. Mean physical-function improvement at two years was 22.4 with surgery and 19.2 with physical therapy, with no significant difference between groups - and this despite 57% of the physical-therapy group crossing over to surgery. The authors call for shared decision-making with full disclosure of the non-surgical evidence.

  4. The only double-blind randomised trial of intradiscal PRP for discogenic low back pain enrolled 47 participants (29 PRP, 18 control) and reported statistically significant improvements in pain, function and satisfaction over EIGHT WEEKS compared with a contrast-agent control. It is a small, single-centre trial, the control group was offered crossover to PRP at 8 weeks, and no disc infection, neurological injury or progressive herniation was reported. The authors themselves call the results promising and say further studies are needed to define who responds.

  5. A follow-up of a SUBSET of patients from that intradiscal PRP trial, contacted once at 5-9 years post-injection, reported statistically and clinically significant improvements in pain and function versus their own baseline. Six patients had gone on to surgery during the interval. This is an uncontrolled long-term look at a subset, not a controlled comparison, and cannot separate treatment effect from natural history.

    Cheng J, Santiago KA, Nguyen JT, Solomon JL, Lutz GE — Treatment of symptomatic degenerative intervertebral discs with autologous platelet-rich plasma: follow-up at 5-9 years.. Regenerative Medicine, 2019. DOI: 10.2217/rme-2019-0040.

  6. A 2026 systematic review and meta-analysis of 15 randomised trials (740 patients) found PRP injections reduced low back pain (SMD -1.32, 95% CI -2.06 to -0.59) and disability (SMD -1.04, 95% CI -1.66 to -0.41) versus controls out to six months. Crucially, the subgroup split went the OTHER way from the marketing: NON-discogenic pain improved consistently at every time point, while DISCOGENIC pain showed benefit only at one month and longer-term effects could not be reliably demonstrated because of high heterogeneity. The authors say the findings must be interpreted cautiously.
